Video diffusion transformers generate high-quality videos, but the quadratic cost of 3D full attention dominates their inference. Dynamic block-sparse attention reduces this cost by computing exact attention only on key/value blocks selected from current activations. However, its fidelity degrades under aggressive sparsity: blocks are scored from coarse summaries or formed by costly global clustering, pruned blocks vanish from the softmax, and whole tiles are kept or dropped even when only a few of their keys matter. To address these limitations, we propose Accurate Block Attention (AB-Attention), a training-free method that accurately approximates dense attention at high sparsity. Its Intra-Block Clustering Selector (IBCS) estimates block importance faithfully yet cheaply by clustering queries only within each query block, without permuting tokens. Sampling with Inclusion-Probability Weighting (SIP) then samples blocks in proportion to these estimates and reweights them by inverse inclusion probability, so that one pass estimates dense attention rather than truncating it. Finally, its kernel gathers key/value blocks much smaller than a tile into full tensor-core tiles, refining selection at no extra tensor-core cost. On Wan2.1 and HunyuanVideo, AB-Attention skips 89--92\% of attention, whereas the baselines compute 1.8--3.6$\times$ as much, yet it still improves PSNR by up to 4.4~dB over the strongest baseline and achieves up to 2.23$\times$ end-to-end speedup. Recent advancements in foundation models (FMs) have catalyzed a paradigm shift in medical image analysis. Unlike traditional task-specific artificial intelligence (AI) models, FMs leverage large-scale datasets to learn generalized representations that can be adapted to downstream clinical applications. Despite the rapid proliferation of FM research in medical imaging, there is a lack of unified synthesis that systematically maps the evolution of architectures, training paradigms, and clinical applications across modalities. To address this gap, this review provides a comprehensive and structured synthesis of FMs in medical image analysis by systematically organizing studies into two primary categories: vision-only foundation models (VFMs) and vision-language foundation models (VLFMs), based on their architectural foundations, training strategies, and downstream clinical tasks. A quantitative analysis was conducted on both VFMs and VLFMs to characterize temporal trends in dataset utilization and application domains, along with pooled performance and subgroup analyses. We also critically discuss persistent challenges, including cross-domain generalization, computational scalability, FM evaluation, fairness, and deployment. Finally, we identify key future research directions aimed at enhancing the robustness, interpretability, and clinical integration of FMs, thereby accelerating their translation into real-world medical practice.

Objective. Deformable image registration remains challenging because corresponding anatomy may exhibit different intensity relationships. Hand-crafted modality-robust descriptors such as Modality-Independent Neighborhood Descriptor with Self-Similarity Context (MIND-SSC) and features from pretrained vision encoders provide distinct representations for correspondence estimation, but how they should be combined remains unclear. We investigate whether tighter integration improves registration over a sequential, scale-separated design. Approach. We introduce FlexiMIND, a training-free and label-free two-stage framework in which frozen FlexiCT-2D encoder features establish contextual correspondence and MIND-SSC refines the deformation within ConvexAdam. Five alternative feature-coupling strategies were evaluated under a protocol across CT-CT (DIR-Lab 4DCT 4DCT, 10 cases), MR-MR (ACDC, 50 patients), and MR-CT (Learn2Reg Abdomen MR-CT, 8 pairs) registration. Performance was assessed using target registration error (TRE), Dice similarity coefficient (Dice), SDlogJ, and local folding. Main results. FlexiMIND achieved a TRE of 1.27 mm on DIR-Lab 4DCT, a DSC of 0.807 on ACDC, and an organ-averaged DSC of 0.825 on Abdomen MR-CT. Across 15 coupling-strategy dataset comparisons, only four outperformed the reference method, with a maximum absolute improvement of 0.0043 in Dice, whereas degradation reached 20.6 % in TRE and 37.9 % in Dice. Changes in registration accuracy were closely associated with changes in deformation-field regularity, and strategies that better preserved regularity generally remained closest to the reference performance. Landmark-level analysis showed that the pretrained encoder stage primarily reduced large correspondence errors rather than uniformly improving local precision. Intrinsic image decomposition aims to separate images into physical components such as albedo, depth, normals, and illumination. While recent diffusion- and transformer-based models benefit from paired supervision from synthetic datasets, their generalization to diverse, real-world scenarios remains challenging. We propose ReasonX, a novel framework that leverages a multimodal large language model (MLLM) as a perceptual judge providing relative intrinsic comparisons, and uses these comparisons as GRPO rewards for fine-tuning intrinsic decomposition models on unlabeled, in-the-wild images. Unlike RL methods for generative models, our framework aligns conditional intrinsic predictors by rewarding agreement between the judge's relational assessments and analytically derived relations from the model's outputs. ReasonX is model-agnostic and can be applied to different intrinsic predictors. Across multiple base architectures and modalities, ReasonX yields significant improvements, including 9-25% WHDR reduction on IIW albedo and up to 46% depth accuracy gains on ETH3D, highlighting the promise of MLLM-guided comparative supervision to bridge low- and high-level vision reasoning.
