Anticipating how users will interact with their surroundings is essential for proactive systems that provide timely assistance in extended reality and embodied applications. Prior work typically forecasts hand-object interactions directly from historical observations, leaving intention and contact geometry implicit. However, similar observation histories can precede semantically or geometrically distinct futures. Inspired by behavioral evidence that gaze can provide early cues for underlying intention, we propose Gaze2HOI, a gaze-aware framework that formulates hand-object interaction forecasting as a hierarchical problem comprising intention inference, contact grounding, and coupled hand-object motion generation. Specifically, Gaze2HOI first weights gaze cues by fixation strength and recency, and uses them to modulate the visual representations of a vision-language model (VLM) through Temporal Gaze Saliency Modulation (TGSM), thereby reducing semantic ambiguity in intention inference. Subsequently, an Anticipatory Contact Grounding (ACG) module maps these structured intentions into explicit contact, support, and placement fields, effectively mitigating geometric ambiguity. Driven by these explicit conditions, a diffusion-based model jointly generates the six-degree-of-freedom (6-DoF) object trajectories and future articulated motion of both hands. To evaluate Gaze2HOI, we further introduce EgoAIM, a phase-aware benchmark that organizes egocentric hand-object interactions into four phases defined by evolving hand-object relations and provides fine-grained intention annotations. Extensive experiments demonstrate that Gaze2HOI achieves state-of-the-art performance in both intention inference and motion forecasting, even approaching oracle-level end-to-end performance in specific interaction phases. Diffusion Transformer (DiT)-based text-to-image models have recently achieved remarkable success in image synthesis. However, when directly applied beyond their training resolution, these models often suffer from severe semantic degradation, where meaningful semantic content disappears. Although existing training-free high-resolution generation methods can mitigate this issue, the inherent semantic degradation at high resolutions remains largely unresolved, leading to degraded fine-grained details. Furthermore, widely used structural guidance methods lack modeling of the structure and details in the intermediate trajectory, which degrades the quality of the generated results. In this paper, we propose a training-free framework for high-resolution image generation to address these challenges. First, we propose a text-visual scale-decoupled attention (TVSDA) module, which restores the model's ability to generate semantically meaningful content at high resolutions by preserving the global receptive field of text tokens while aligning the receptive field of image tokens with the training resolution. Second, we propose a structure-detail guidance (SDG) strategy that explicitly models structural information and fine details through scale-wise and temporal-wise guidance, overcoming the limitations of conventional low-resolution guidance. Extensive experiments demonstrate that our method significantly improves both structural coherence and detail fidelity over existing training-free DiT-based high-resolution generation approaches. Medical large Vision-Language Models (Med-LVLMs) have shown strong potential in multimodal clinical applications such as medical visual question answering and report generation. However, Med-LVLMs remain challenged by hallucinations caused by modality misalignment, where models prioritize textual knowledge over visual evidence and generate outputs that conflict with medical images. To mitigate this issue, recent studies have explored preference optimization to improve image–text alignment, achieving promising results. Despite these advances, existing preference-based methods still face two limitations in medical settings: (1) overfitting to superficial cues, and (2) pseudo convergence of the preference signal. In this paper, we propose Dynamic Evidence-Guided Preference Optimization (DEPO), a new framework that enables evidence-aware and adaptive preference learning for Med-LVLMs. DEPO introduces Multi-Modal Evidence Perturbation (MEP) to suppress non-causal textual and visual shortcuts, and Dispreferred Evidence Resampling (DER) to continuously update dispreferred responses as hallucination patterns evolve. Experiments on multiple medical VQA and report generation benchmarks demonstrate consistent improvements over existing methods, with strong robustness across datasets and architectures. All Codes and data will be released after review.

Reinforcement learning with verifiable rewards (RLVR) has significantly advanced the reasoning ability of vision-language models (VLMs). However, the inherent text-dominated nature of VLMs often leads to insufficient visual faithfulness, characterized by sparse attention activation to visual tokens. More importantly, our empirical analysis reveals that temporal visual forgetting along reasoning steps exacerbates this deficiency. To bridge this gap, we propose Visually-Guided Policy Optimization (VGPO), a novel framework to reinforce visual focus during policy optimization. Specifically, VGPO initially introduces a Visual Attention Compensation mechanism that leverages visual similarity to localize and amplify visual cues, while progressively elevating visual expectations in later steps to counteract visual forgetting. Building on this mechanism, we implement a dual-grained advantage re-weighting strategy: the intra-trajectory level highlights tokens exhibiting relatively high visual activation, while the inter-trajectory level prioritizes trajectories demonstrating superior visual accumulation. Extensive experiments demonstrate that VGPO achieves better visual activation and superior performance in mathematical multimodal reasoning and visual-dependent tasks. The code has been released at https://github.com/wzb-bupt/VGPO.
