A 3D foundation model unifying CT interpretation and segmentation

原文摘要

Radiologists interpret computed tomography (CT) scans by jointly determining what a finding represents and where it lies in three-dimensional (3D) anatomy. However, CT models have treated CT interpretation and 3D segmentation as separate tasks, leaving textual findings spatially unverifiable and overlooking potential synergy between clinical understanding and spatial perception. Here we present HoloCT, a generalist 3D foundation model unifying CT interpretation and voxel-level segmentation by jointly modelling whole-volume context and fine-grained anatomical detail, providing 3D masks as evidence. We curated HoloDATA, comprising 195,921 CT scans from 73 public datasets and six clinical cohorts, with 6.2 million instruction dialogues and 2.8 million CT-mask pairs. Across 346 tasks spanning segmentation, lesion screening, visual question answering, and report generation on 61 internal and 12 external datasets, HoloCT outperformed specialist and foundation models. In multicentre validation across 11,455 clinical CT scans from five hospitals, HoloCT outperformed competing models in lesion segmentation and report generation by 18.2 and 15.3 points. It also achieved 85.1% mean accuracy in screening thyroid tumours and colon lesions, exceeding all four participating radiologists in a retrospective reader study. Further analyses revealed bidirectional semantic-spatial synergy: language-guided segmentation delineated unseen anatomical structures and lesions as well as diffuse free-text findings, outperforming the leading approach by 22.1 points, whereas prompting the model to generate corresponding 3D masks alongside its textual outputs significantly improved interpretation performance. HoloCT shows that clinical understanding and spatial perception can be learned together within a single foundation model and offers a path towards more accurate and reliable CT analysis.

2AM: Grounding Agent-Side Memory as Guidance for Steerable Action Models in Long-Horizon Manipulation

Yutong Hu, Fengjiao Chen, Xuezhi Cao, Renaud Detry

原文摘要

Long-horizon robot manipulation requires memory, but not necessarily inside the action policy. To address such tasks, current agentic systems often combine VLAs with planners and geometric tools, sometimes using additional depth or calibrated geometry. These systems confound attribution: gains may come from richer observations or alternative motor tools, while failures may stem from either the policy or an under-specified language interface. We isolate this question through a deliberately constrained design: less tool breadth, but greater interface bandwidth. 2AM makes a multimodal Agent the sole holder of task memory and a single RGB-based, episodically stateless Action Model the sole executor of task-relevant motion. The Agent compiles interaction history into subtask language and optional 2D grasp, place, and move hints that bind its physical intention at different time scales. To teach this steerability to the VLA, we augment demonstrations with structured hint labels and train under condition dropout, spatial noise, and temporal jitter to tolerate imperfect Agent outputs. On LIBERO-Mem, without depth, online geometry, or planner-based object motion, 2AM reaches 76.3% average completion, a 61.5-point improvement over the strongest reported baseline of 14.8%, together with 63.0% relaxed and 11.8% strict success. These results show that task memory can remain Agent-side. They further show that Action Model capability depends not only on what the policy has learned, but on how precisely the Agent can steer it. Reasoning models think in long, unstructured streams with no mechanism for compressing or organizing their own intermediate state. We introduce MEMENTO: a method that teaches models to segment reasoning into blocks, compress each block into a memento, i.e., a dense state summary, and reason forward by attending only to mementos, reducing context, KV cache, and compute. To train MEMENTO models, we release OpenMementos, a public dataset of 228K reasoning traces derived from OpenThoughts-v3, segmented and annotated with intermediate summaries. We show that a two-stage SFT recipe on OpenMementos is effective across different model families (Qwen3, Phi-4, Olmo 3) and scales (8B--32B parameters). Trained models maintain strong accuracy on math, science, and coding benchmarks while achieving ${\sim}2.5\times$ peak KV cache reduction. We extend vLLM to support our inference method, achieving ${\sim}1.75\times$ throughput improvement while also enabling us to perform RL and further improve accuracy. Finally, we identify a dual information stream: information from each reasoning block is carried both by the memento text and by the corresponding KV states, which retain implicit information from the original block. Removing this channel drops accuracy by 15\,pp on AIME24.
