Morphological Decoupling-Based Skeletal Classification for Clinical Assessment of Malocclusion

Zhichun Jin, Zhicheng He, Hao Xu, Dongyang Li, Lin Wang, Hongliang Ren, Long Bai

Malocclusion skeletal grading is a fundamental task in orthodontics, critical for diagnosis and treatment planning. Traditionally, cone-beam computed tomography (CBCT) is used for visual measurement, and the reconstructed lateral cephalograms are handed over to expert dentists for diagnosis. However, manual review is time-consuming, labor-intensive, and subject to inter-operator variability. Therefore, an automatic CBCT-based system is needed for reliable malocclusion skeletal grading. In this case, we develop TeethGNN, a novel graph-based framework designed to combine CBCT image features with morphological information for accurate and efficient malocclusion grading. TeethGNN utilizes a decoupled learnable decoder to directly predict key morphological indicators from CBCT images, eliminating the need for manual measurements. These morphological features are then fused with image features using a graph neural network (GNN), which effectively models the relationships between the modalities. To further enhance robustness and calibration, we introduce a collaborative calibration strategy. This strategy combines multi-scale graph adversarial perturbation for explicit calibration and nonlinear topological graph calibration for implicit confidence adjustment. Extensive experiments and ablation studies on our collected clinical dataset demonstrate that our malocclusion measurement system achieves 77.08\% in accuracy and 89.61\% in AUC, outperforming the compared state-of-the-art methods. These results validate the effectiveness of graph-based multimodal fusion and collaborative calibration in improving malocclusion grading performance. Our system shows strong potential for advancing computer-aided orthodontic diagnosis, providing an accurate and reliable solution for vision-based clinical measurement and diagnosis.

Dual Exam: Controlled Evaluation of Multimodal Agents and World Models

Zhefan Rao, Meng Chu, Xuan Billy Zhang, Jingfan Chen, Guohao Cai, Runyi Li, Shiyi Du, Xu Huang 等 92 位作者

World Models (WMs) can support Multimodal Agents (MAs) through simulated experience, while Agent trajectories can inform World Model refinement. Evaluating this interaction requires separate evidence about action selection and state evolution. We introduce Dual Exam, which tests Agents under fixed environment response mechanisms and World Models under fixed action interventions across physical, digital, social, and scientific domains. The Agent track compares 10 configurations on 1,088 items in 15 reporting categories. The World Model track contains 400 cases across eight scenarios; 15 configurations have full coverage and one covers six scenarios. GPT 6 Astra leads the case-weighted Agent Index at 31.3, and GPT-Image-2 leads the full-coverage WM Overall at 21.7. These scores use different tasks and metrics and are not directly comparable. A 150-case pilot of one coupled pairing records zero task successes and motivates component-level diagnosis. Results reveal uneven task performance and failures to preserve task-relevant state. Their value for predicting coupled success or sustained reciprocal learning remains to be established.

Look Beyond the Question: Preserve-Focus-Fuse for Whole-Slide Image Question Answering

A question about a whole-slide image rarely names every morphological finding needed to answer it. Question-guided patch selection can therefore overlook supporting morphology, while slide-level compression can dilute focal evidence. Inspired by how pathologists combine broad slide review, question-directed examination, and integration of their findings, we introduce BEYONDPATH, a Preserve-Focus-Fuse framework for whole-slide image question answering. Preserve learns a question-agnostic Morphology Bank through answer supervision across multiple questions about each training slide. Focus retrieves questionspecific evidence from all foreground patches, including regions outside the Bank. Fuse integrates the preserved morphology at the retrieved evidence positions through Morphology-on-Visual-Evidence Fusion, producing a fixed 96-token visual prefix. On WSI-Bench, BEYONDPATH achieves an Avg-9 of 0.769 and leads all eight reported report-generation metrics using the same checkpoint. It also achieves 0.639 zero-shot overall accuracy on SlideBench-BCNB. Controlled ablations show complementary benefits from morphology preservation and question-directed retrieval, and support cross-question supervision and fusion at the evidence positions. These results support looking beyond the question's explicit cues: preserving broader slide morphology while retaining the ability to seek evidence for each question.

Rethinking Data Imbalance in Class Incremental Surgical Instrument Segmentation

Shifang Zhao, Long Bai, Kun Yuan, Feng Li, Jieming Yu, Wenzhen Dong, Guankun Wang, Mobarakol Islam 等 11 位作者

In surgical instrument segmentation, the increasing variety of instruments over time poses a significant challenge for existing neural networks, as they are unable to effectively learn such incremental tasks and suffer from catastrophic forgetting. When learning new data, the model experiences a sharp performance drop on previously learned data. Although several continual learning methods have been proposed for incremental understanding tasks in surgical scenarios, the issue of data imbalance often leads to a strong bias in the segmentation head, resulting in poor performance. Data imbalance can occur in two forms: (i) class imbalance between new and old data, and (ii) class imbalance within the same time point of data. Such imbalances often cause the dominant classes to take over the training process of continual semantic segmentation (CSS). To address this issue, we propose SurgCSS, a novel plug-and-play CSS framework for surgical instrument segmentation under data imbalance. Specifically, we generate realistic surgical backgrounds through inpainting and blend instrument foregrounds with the generated backgrounds in a class-aware manner to balance the data distribution in various scenarios. We further propose the Class Desensitization Loss by employing contrastive learning to correct edge biases caused by data imbalance. Moreover, we dynamically fuse the weight parameters of the old and new models to achieve a better trade-off between the biased and unbiased model weights. To investigate the data imbalance problem in surgical scenarios, we construct a new benchmark for surgical instrument CSS by integrating datasets. Extensive experiments demonstrate the effectiveness of the proposed framework, achieving significant performance improvement against existing baselines.

EvolveMAS: A Unified Self-Evolution Framework for Decentralized Multi-Agent Systems

Yalun Wu, Qiwei Ma, Bingbing Xu, Dan Zhang, Long Bai, Boyang Wang, Jiatong Li, Zheqi Lv 等 13 位作者

Decentralized multi-agent systems (MAS) enable large language model agents to interact without centralized control. Agents need to adapt to other agents at test time and reuse accumulated experience to refine their interaction strategies for future interactions across cooperative, mixed-motive, and adversarial settings. However, existing approaches often develop adaptation mechanisms for particular interaction settings. This raises a central question: how can decentralized MAS support test-time adaptation and reliable experience reuse across interaction settings? We propose EvolveMAS, a unified self-evolution framework that supports experience-driven adaptation within and across episodes. EvolveMAS pairs each main agent with a dedicated MemoryAgent for test-time memory governance, which maintains isolated Structured Memory outside the main agent's reasoning context. EvolveMAS organizes adaptation into two complementary processes: Within-Episode Online Adaptation and Cross-Episode Knowledge Evolution. During Within-Episode Online Adaptation, MemoryAgent updates episodic, semantic, and working memory from interaction evidence to inform subsequent actions. During Cross-Episode Knowledge Evolution, the main agent uses memory and episode outcomes to distill reusable interaction knowledge comprising partner profiles and interaction strategies for subsequent episodes. Across five benchmarks and six evaluation configurations spanning the three interaction settings, EvolveMAS achieves the highest normalized average score of 77.2 among the compared methods. Ablations show that test-time memory governance and cross-episode knowledge reuse both improve performance. Interaction traces further illustrate how updated memory and distilled knowledge guide subsequent interactions. These results support experience-driven adaptation in decentralized MAS without updating model parameters.

Maximizing Local Entropy Where It Matters: Prefix-Aware Localized LLM Unlearning

Naixin Zhai, Pengyang Shao National University of Singapore, Binbin Zheng University of Science and Technology of China, Yonghui Yang National University of Singapore, Fei Shen National University of Singapore and Tencent, Long Bai Institute of Computing Technology, Chinese Academy of Sciences, Xun Yang University of Science and Technology of China

Machine unlearning aims to forget sensitive knowledge from Large Language Models (LLMs) while maintaining general utility. However, existing approaches typically treat all tokens in a response indiscriminately and enforce uncertainty over the entire vocabulary. This global treatment results in unnecessary utility degradation and extends optimization to content-agnostic regions. To address these limitations, we propose PALU (Prefix-Aware Localized Unlearning), a framework driven by a local entropy maximization objective across both temporal and vocabulary dimensions. PALU reveals that (i) suppressing the sensitive prefix alone is sufficient to sever the causal generation link, and (ii) flattening only the top-K logits is adequate to maximize uncertainty in the critical subspace. These findings allow PALU to alleviate redundant optimization across the full vocabulary and parameter space while minimizing collateral damage to general model performance. Comprehensive evaluations validate that PALU achieves superior forgetting efficacy and utility preservation compared to state-of-the-art baselines. Our code is available at https://github.com/nxZhai/PALU.
