Both full fine-tuning (Full FT) and parameter-efficient fine-tuning methods such as LoRA introduce weight updates without accounting for the spectral structure established during pretraining. As a result, noisy gradients from limited fine-tuning data can perturb robust pretrained features. We identify spectral preconditioning as the missing ingredient: reparameterizing each weight matrix through its full-rank singular value decomposition (SVD) and freezing one singular basis constrains updates to the pretrained column space, yielding a preconditioned optimization scheme that outperforms unconstrained Full FT at the same trainable parameter count. Building on this insight, we propose FuRA (Full-Rank Adaptation), an efficient full-rank adaptation framework based on a block tensor-train factorization W = LSR, where the large core L is fixed to the pretrained block-wise SVD basis, while only the compact core R and the block-wise singular values S are optimized. This design simultaneously provides full-rank spectral preconditioning, preserves full-rank update expressivity, and achieves parameter, memory, and step-time efficiency comparable to LoRA. FuRA consistently outperforms Full FT across multiple settings, including LLM fine-tuning (+1.37 on LLaMA-3-8B commonsense reasoning), LLM reinforcement learning for mathematical reasoning, and visual instruction tuning for VLMs. Furthermore, the 4-bit quantized variant, QFuRA, also surpasses QLoRA. Offline reinforcement learning and off-policy evaluation evaluate dynamic decision rules based on historical data, without the ability to deploy online. In recent artificial intelligence (AI) applications, simple state and reward information is recorded as complex text or image, expert annotation may be available but at a higher cost. Recent AI and machine learning (ML) predictors can label data cheaply but with unknown bias. We show how a limited budget for ground-truth data-annotation can safely make use of plentiful ML predictions in a new estimator for doubly-robust off-policy evaluation with missing rewards. Our method directs where data should be collected by optimizing annotation probabilities for the best downstream estimation variance, which we implement in a feasible batch-adaptive approach. Our work is motivated by a collaboration with a homelessness services nonprofit that writes casenotes for individuals over time. Our method can be used to unlock trustworthy inference from casenote data and answer new inferential questions such as: how does expanding outreach effort over time affect progress towards a housing application and improvement in housing placement? In simulations and on two real datasets -- casenotes from the nonprofit and human-preference votes from LMArena -- we see reductions in RMSE of 37-65% for housing placement and 21-67% for progress towards a housing application at budgets of 50% of full annotation and above, and by 55-62% at every budget on LMArena under an equal-exposure policy.

Intelligent education systems often collect exam sheets as in-the-wild photos. These photos often suffer from distortions and noise caused by handwriting and occlusions, collectively referred to as Real-World Degraded Exam Images (RDEI). Structure-preserving reconstruction is key to converting RDEI into structured assets for downstream educational applications. Existing Multimodal Large Language Models (MLLMs) often fail under RDEI, leading to disrupted structure and evidence-unsupported hallucinations. To tackle these challenges, we propose MessToClean, a backbone-agnostic, evidence-driven pipeline that treats off-the-shelf MLLMs as interchangeable components. By grounding extraction in pixel-aligned evidence and enforcing post-hoc consistency auditing on recovered structures, MessToClean mitigates unsupported hallucinations and enhances both controllability and structural fidelity in question-level reconstruction. We curate RDEI-Exam from our educational platforms and evaluate across 12 state-of-the-art MLLM backbones. Across these, MessToClean improves stem consistency by 1.01-3.18%, figure consistency by 0.50-49.16%, and refusal F1 by 1.06-10.88% across question types.
