OCR in the Foundation Model Era: From Specialist Models to Unified Models

原文摘要

Optical Character Recognition (OCR) is among the oldest and most widely deployed applications of artificial intelligence. This survey reviews its development, focusing on the Foundation Model Era. We define OCR through faithful transcription: within a specified visual scope, OCR should completely reconstruct the text, notation, and document structure observable in the source image, and every textual, notational, or structural element in the output must be verifiable from visual evidence in that image. We trace the development from specialist OCR models toward unified models, examining joint text detection and recognition, structured sequence generation, and the use of pretrained vision-language models (VLMs). We organize contemporary OCR systems into five coexisting families: modular toolkits and task-specific models, specialized document VLMs, hybrid workflows, general-purpose VLMs, and commercial products and managed services. We compare their architectures, data, and training, examining hallucination and omission, insufficient spatial grounding, and inference cost in generative OCR. We review evaluation from character and scene text recognition to parsing individual structural elements and full-page transcription, highlighting gaps in consistent output definitions, attribution of performance to individual components, and coverage of real capture conditions and visible content and structure. Finally, we summarize the challenges facing OCR and discuss directions for future research. FermatSyn: SAM2-Enhanced Bidirectional Mamba with Isotropic Spiral Scanning for Multi-Modal Medical Image Synthesis

Feng Yuan, Yifan Gao, Haoyue Li, Xin Gao

原文摘要

Multi-modal medical image synthesis is pivotal for alleviating clinical data scarcity, yet existing methods fail to reconcile global anatomical consistency with high-fidelity local detail. We propose FermatSyn, which addresses three persistent limitations: (1) SAM2-based Prior Encoder that injects domain-aware anatomical knowledge via LoRA$^{+}$ efficient fine-tuning of a frozen SAM2 Vision Transformer; (2) Hierarchical Residual Downsampling Module (HRDM) coupled with a Cross-scale Integration Network (CIN) that preserves high-frequency lesion details and adaptively fuses global--local representations; and (3) continuity constrained Fermat Spiral Scanning strategy within a Bidirectional Fermat Scan Mamba (BFS-Mamba), constructing an approximately isotropic receptive field that substantially reduces the directional bias of raster or spiral serialization. Experiments on SynthRAD2023, BraTS2019, BraTS-MEN, and BraTS-MET show FermatSyn surpasses state-of-the-art methods in PSNR, SSIM, FID, and 3D structural consistency. Downstream segmentation on synthesized images yields no significant difference from real-image training ($p{>}0.05$), confirming clinical utility. Code is available at https://github.com/exsinger-hub/ FermatSyn.

原文摘要

Recent retrieval-augmented generation (RAG) approaches have demonstrated strong capability in handling complex queries, yet current research overlooks a critical challenge: different retrievers require fundamentally different query formulation strategies for optimal performance. In this work, we present the first systematic analysis of how LLMs can learn to adapt their query formulation strategies for different retrievers via reinforcement learning (RL). Our empirical study reveals that RL effectively teaches an LLM to tailor its queries to specific retriever characteristics. We discover that different retrievers exhibit surprisingly distinct optimal query styles (e.g., descriptive vs. question-like), suggesting strategies learned for one retriever ineffective for another. We further show that performance can be enhanced by incorporating retriever-specific human guidance and by scaling model size. To facilitate learning over multi-retrieval-step trajectories, we introduce a branching-based rollout technique that improves training stability. Our work provides the first empirical evidence and actionable insights for building truly retriever-aware RAG systems. CT-FineBench: A Diagnostic Fidelity Benchmark for Fine-Grained Evaluation of CT Report Generation

原文摘要

The evaluation of generated reports remains a critical challenge in Computed Tomography (CT) report generation, due to the large volume of text, the diversity and complexity of findings, and the presence of fine-grained, disease-oriented attributes. Conventional evaluation metrics offer only coarse measures of lexical overlap or entity matching and fail to reflect the granular diagnostic accuracy required for clinical use. To address this gap, we propose CT-FineBench, a benchmark built from CT-RATE and Merlin to evaluate the fine-grained factual consistency of CT reports, constructed from CT-RATE and Merlin. Our benchmark is constructed through a meticulous, Question-Answering (QA) based process: first, we identify and structure key, finding-specific clinical attributes (e.g., location, size, margin). Second, we systematically transform these attributes into a QA dataset, where questions probe for specific clinical details grounded in gold-standard reports. The evaluation protocol for CT-FineBench involves using this QA dataset to query a machine-generated report and scoring the correctness of the answers. This allows for a comprehensive, interpretable, and clinically-relevant assessment, moving beyond superficial lexical overlap to pinpoint specific clinical errors. Experiments show that CT-FineBench correlates better with expert clinical assessment and is substantially more sensitive to fine-grained factual errors than prior metrics.
