Perception Over Geometry: Rethinking Gaussian Allocation in Mesh-based Splatting

Yuting Wang, Yang Zhao, Shanding Diao, Penglin Wang, Yuan Chen, Wei Jia, Ronggang Wang

原文摘要

3D Gaussian Splatting (3DGS) enables photorealistic novel-view synthesis but lacks explicit topology for geometric editing. Existing mesh-based methods enable geometric manipulation by associating Gaussians with triangular mesh faces, typically assigning a fixed number of Gaussians to each face. However, the representation capacity required for appearance reconstruction depends on local appearance complexity rather than merely on mesh tessellation density. To address these limitations, we propose P-MGS, a framework that separates appearance modeling from geometric control to achieve high quality rendering, compact representation, and flexible mesh editing. P-MGS introduces a perceptual complexity guided allocation strategy that distributes Gaussians according to surface appearance demand estimated from multiview observations. To preserve reconstruction flexibility, Gaussians are optimized without mesh constraints and subsequently associated with the original mesh through post hoc binding, retaining adaptive refinement while preserving mesh topology. During mesh editing, local affine deformation transfer updates Gaussian centers and covariances under mesh deformation, reducing misalignment and floating artifacts. Experiments demonstrate superior average novel view synthesis quality with fewer Gaussians and coherent mesh editing.

In this work, we evaluate a simple inference-time scaling approach based on \textit{turn-level best-of-N sampling} (\method) for large language model (LLM) agents on complex, long-horizon tasks. We instantiate this using a \textit{generator-selector} framework with two LLMs; at each turn, a \emph{generator} model generates $N$ candidate actions and a \emph{selector} model selects one of them, which is then executed in the environment. This paper is closely related to the work of \cite{zainullina2025guided} who used critic models for guided search at inference time. Different from their work, we evaluate if off-the-shelf, open-weight LLMs can be effective at comparing different candidate actions directly. Experiments on long-horizon terminal tasks show that \method can lead to significant performance improvements, both in terms of accuracy (5--10\%) and consistency across repeated attempts. Analysis of agent trajectories reveal a shift in agent behaviour towards desirable patterns such as reduction in number of attempts to solve the task, improved task instruction following, debugging and testing.

Verified Implementation of the Next-gen Borrow Checking Algorithm for Rust via Abstract Interpretation

Jinhua Wu, Yuting Wang, Zhemin Chen

原文摘要

Borrow checking is a central mechanism for Rust to ensure temporal memory safety. Its correctness is based on the intuition that the Aliasing XOR Mutability discipline guarantees that programs without borrow errors inherently could not access invalid references (i.e., pointers). However, existing work on formalizing borrow checking treats it either as a component of type systems or as symbolic execution describing resource transfer. None provides a direct explanation of the correctness of borrow checking algorithms such as Non-Lexical Lifetimes (NLL) or Polonius in the official Rust compiler that function more like static pointer analysis. We take a first step toward solving this problem by formalizing Polonius, the next-generation borrow- checking algorithm for the Rust compiler, on a core subset of Rust. We prove its correctness by showing that any program accepted by Polonius does not access invalid references at runtime. The key observation is that, when borrow checking is performed lazily, Polonius could be treated as a forward abstract interpretation on abstract domains recording pointer aliasing (borrowing) relations. By providing a concrete semantics that formally describes the Aliasing XOR Mutability discipline, we prove the soundness of this abstract interpretation, from which we derive memory safety of checked programs. This formulation of Polonius is implemented and formally verified in the abstract interpretation framework of CompCert. The correctness proofs are generated by an agentic AI under human guidance using paper proofs as proof outline. To the best of our knowledge, this is the first comprehensive formalization and verification of a borrow checking algorithm as manifested in the Rust compiler.

Separating Common and Unique Directions for Model Merging

Shiting Wang, Yingjie Zou, Ke Li

原文摘要

Model merging combines multiple fine-tuned models without additional training, and arithmetic methods have emerged as a simple yet effective way to merge models without data. However, existing arithmetic methods treat task vectors in parameter space and apply a single element-wise rule to directions that can differ in mergeability. In a shared directional basis, we find a consistent two-regime structure: a low-dispersion common bulk shared across tasks and a high-dispersion task-dominated tail that carries disproportionate energy. These findings motivate Common--Unique Subspace Decomposition (CUSD), a data-free pre-merging method that separates task vectors into common and unique directional components, merges the common components with a downstream arithmetic operator, and retains unique residuals separately. Experiments across $11$ base models, four model families, and four arithmetic-based merging operators show that CUSD improves 42 of 44 model--operator combinations and achieves up to 22.5% improvement over the corresponding baselines. Our code is publicly available at https://github.com/stwang666/CUSD

Safety, or Just Capability? A Validity Audit of Agent-Safety Benchmarks

Youting Wang, Xiao Han, Dingyan Shang, Yuan Tang, Bowen Liu

原文摘要

Agent-safety benchmarks measure different behaviors, and their scores get quoted interchangeably as an agent's safety. We treat four of them (R-Judge, InjecAgent, AgentHarm, AgentDojo) as measurements to be validated, running each under its official implementation and author-provided scorer on up to 22 models, with MMLU and GPQA measured by us under one protocol as a capability composite. The metric is the first problem. On any binary trace-judgment benchmark scored by $F_1$, an ``always positive'' policy attains $F_1 = 2π/(1+π)$; on R-Judge that is $0.690$, above five of the 21 models that actually discriminate. The three broad-coverage benchmarks then rank the same 18 models differently, and the trade-off behind that disagreement is a small-panel artifact: R-Judge specificity against AgentHarm safety correlates $-0.64$ at $n{=}7$ and $+0.02$ at $n{=}18$, and a quarter of random size-7 subsets reach $|ρ| \geq 0.5$ around that near-zero value. Held-out validity turns on which outcome you pick. Capability predicts task success ($ρ{=}{+}0.60$) but correlates negatively with misalignment safety ($ρ{=}{-}0.44$, $n{=}21$). On their paired $n{=}20$ panel, the corresponding contrast is $Δ{=}{-}1.00$ (95% CI $[-1.48, -0.49]$, $p<0.001$), and it survives leave-one-organization-out and organization-clustered bootstrap analyses. On an expanded 41-model panel, the misalignment correlation weakens to $-0.16$ (95% CI $[-0.54, +0.22]$) and jailbreak strengthens to $+0.34$, though neither change is significant. \mbox{AgentHarm} shows the strongest held-out association, $ρ{=}{+}0.72$ with three-template jailbreak safety after controlling capability. But both instruments score harmful compliance, so this is evidence of convergent validity rather than general safety. Naming the benchmark, metric, target behavior, and model panel is the minimum a safety claim needs.

原文摘要

On-device RGB photo enhancement is a popular task that is needed for quality improvement of photos captured with mobile devices as well as for retouching of images acquired through other sources. This creates a need for efficient deep learning models for this problem that are not only performant but can also be deployed on constrained mobile hardware. In this Mobile AI challenge, we address this problem and propose participants to design fast solutions that can perform comprehensive enhancement of RGB photos while running on popular smartphone GPUs from Qualcomm and Arm. For this, the participants were provided with a large-scale DPED dataset consisting of RGB image pairs captured with an old iPhone 3GS smartphone and a professional Canon 70D DSLR camera. The runtime of all models was evaluated on the Snapdragon 8 Elite Gen 5 and Dimensity 9500 chipsets with the Adreno and Mali GPUs, respectively. Top solutions proposed in this challenge are able to enhance Full HD resolution RGB images on both platforms under 300 milliseconds. A comprehensive description of the models developed in the challenge is provided in this paper.

OpenEBFM: Energy-based Foundation Models Are Scalable Architecture

Yuan Pu, Yudong Lu, Ting Wang, Zhicong Hong, Qinuo Li, Xuansheng Wu, Na Zou, Xia Hu 等 9 位作者

原文摘要

Energy-based models have shown promising properties for domain-specific tasks but remain challenging in modeling general-purpose scenarios. To bridge this gap, we design energy-steered models (ESMs) for foundational language modeling. ESMs integrate energy steering and token generation within a single architecture to leverage the energy-guided search to refine token generation. Specifically, ESMs first optimize a latent state over the energy landscape, then leverages the refined latent state for token generation. By performing such energy-guided pre-training on large-scale corpora, ESMs learn generally effective energy landscape where favorable tokens correspond to lower energy values. To explore the scaling behavior of ESMs, we pre-train ESMs on three large-scale corpora, then evaluate across 26 downstream tasks spanning a broad range of domains. Our results demonstrate that ESMs have promising scaling behaviors with increasing model size and training data, and outperform GPTs on most downstream tasks. These findings highlight ESMs as a promising architecture for large-scale pre-training and general purpose problem solving. We fully release the model weights and training infrastructure to enable the community to completely reproduce and extend our results. MUSE: Multi-Level Memory with Selective Runtime Injection for Scientific Coding Agents

Haoran Jin, Defu Lian, Xiting Wang, Hong Xie

原文摘要

Historical experience can guide scientific coding at multiple levels, but task-level retrieval may miss implementation lessons that become relevant during execution. We introduce MUSE, a memory management framework combining multi-level memory with selective runtime injection for scientific coding agents. MUSE builds global, approach-level, and implementation-level memory from historical trajectories. At initialization, the agent receives global memory and problem-matched approach-level memory. During execution, current interactions are used to retrieve implementation-level memory. An LLM gate selects relevant experience and generates specific review advice, or skips injection. Across four benchmarks spanning numerical problem solving, kernel optimization, and scientific software repair, MUSE outperforms retrieval and memory baselines on all evaluation metrics. Ablations support the multi-level design and gated injection, while transfer experiments show that memory remains useful across kernel benchmarks. These results show that multi-level memory and selective runtime injection enable effective reuse of experience in scientific coding. Code is available at https://github.com/hr-jin/MUSE.

Metacognitive Decoding: Efficient Inference for Diffusion Language Agents

Mingyu Zhao, Shuhang Lin, Ligong Han, Xiaojing Yu, Zhenting Wang, Yifei Pang, Yang Zhou, Dimitris N. Metaxas

原文摘要

Diffusion language models offer a promising path to efficient agents through parallel token generation. However, efficient interaction requires adapting both where computation is spent and how much text is generated. We introduce a metacognitive controller that guides diffusion generation according to the needs of the current response and the agent's interaction with the environment. It learns when to finalize tokens and how far generation should extend, reducing overall computation within responses. Our controller, a lightweight plug-in for the diffusion backbone, is trained on agent trajectories to balance correctness and generation cost. This approach connects the agent's generation process to the interaction the agent serves. Experiments across five benchmarks demonstrate that our approach reduces inference FLOPs by 20.6% on average compared with the most efficient baseline, while maintaining competitive task accuracy. Ablation studies further validate the effectiveness of our approach.

A Survey of Inductive Reasoning for Large Language Models

Kedi Chen, Dezhao Ruan East China Normal University, Yuhao Dan, Yaoting Wang Fudan University, Siyu Yan, Xuecheng Wu, Yinqi Zhang Xiaohongshu, Qin Chen East China Normal University 等 15 位作者

原文摘要

Reasoning is an important task for large language models (LLMs). Among all the reasoning paradigms, inductive reasoning is one of the basic types, which is characterized by its particular-to-general thinking process and the non-uniqueness of its answers. The inductive mode is crucial for knowledge generalization and aligns better with human cognition, so it is a fundamental mode of learning, hence attracting increasing interest. Despite the importance of inductive reasoning, there is no systematic summary of it. Therefore, this paper presents the first comprehensive survey of inductive reasoning for LLMs. First, methods for improving inductive reasoning are categorized into three main areas: post-training enhancement, test-time exploration, and data augmentation. Then, current benchmarks of inductive reasoning are summarized, and a unified sandbox-based evaluation approach with the observation coverage metric is derived. Finally, we offer some analyses regarding the source of inductive ability and how simple model architectures and data help with inductive tasks, providing a solid foundation for future research.

原文摘要

Supervised Fine-Tuning (SFT) and Reinforcement Learning (RL) dominate the post-training landscape for mathematical reasoning, yet differ fundamentally in their reliance on expert trajectories. To understand the optimal way to harness these trajectories for maximizing performance, we propose the Plasticity-Ceiling Framework. This framework empirically grounds the post-training landscape by decomposing the final performance ceiling into the foundational SFT performance and the subsequent RL plasticity (i.e., the maximum improvement via RL). Through extensive benchmarking, we establish the Sequential SFT-then-RL pipeline as the superior standard, overcoming the stability and premature convergence deficits inherent in synchronized approaches. Furthermore, we derive precise scaling guidelines: (1) Transitioning to RL at the Stable or Mild Overfitting Regime of SFT maximizes the final ceiling by securing a robust SFT foundation with substantial RL plasticity; (2) Refuting the “Less is More” hypothesis in SFT-then-RL scaling, we demonstrate that Data Scale determines the primary post-training potential, while Trajectory Difficulty acts as a performance multiplier; and (3) The Minimum Validation Loss of SFT serves as a reliable indicator for selecting the expert trajectories that maximize the ultimate performance ceiling. Our findings provide actionable guidelines for extracting maximum value from expert trajectories.

PIArena: A Platform for Prompt Injection Evaluation

Runpeng Geng, Chenlong Yin Pennsylvania State University, Yanting Wang, Ying Chen Pennsylvania State University, Pennsylvania State University, Jinyuan Jia Pennsylvania State University

原文摘要

Prompt injection attacks pose serious security risks across a wide range of real-world applications. While receiving increasing attention, the community faces a critical gap: the lack of a unified platform for prompt injection evaluation. This makes it challenging to reliably compare defenses, understand their true robustness under diverse attacks, or assess how well they generalize across tasks and benchmarks. For instance, many defenses initially reported as effective were later found to exhibit limited robustness on diverse datasets and attacks. To bridge this gap, we introduce PIArena, a unified and extensible platform for prompt injection evaluation that enables users to easily integrate state-of-the-art attacks and defenses and evaluate them across a variety of existing and new benchmarks. We also design a dynamic strategy-based attack that adaptively optimizes injected prompts based on defense feedback. Through comprehensive evaluation using PIArena, we uncover critical limitations of state-of-the-art defenses: limited generalizability across tasks, vulnerability to adaptive attacks, and fundamental challenges when an injected task aligns with the target task. The code and datasets are available at https://github.com/sleeepeer/PIArena.

SOAR: Supervision from Observation for Agentic Reinforcement Learning

Meng Li, Lei Li Tencent, Xiting Wang Renmin University of China, Yi Yuan, Zheng Wei Tencent Technology (Beijing) Company Limited, Brucebian Tencent AI Lab, Zang Li Tencent

原文摘要

Agentic reinforcement learning enables large language models to solve long-horizon tasks by interacting with the environment and internalizing tool-use behavior into their reasoning. Prior work assigns supervision primarily based on outcome rewards or external reward models, but largely ignores environment observations, a critical source of learning. Consequently, agents may identify successful actions without understanding how the environment responds, producing suboptimal policies. To address this, we propose SOAR (Supervision from Observation for Agentic Reinforcement Learning), which assigns positive advantages to observation tokens proportional to the negative entropy of preceding actions. This encourages the agent to learn from outcomes of confident actions, grounding policy updates in environment dynamics and improving anticipation of tool-call consequences. Empirical results across three domains and 14 benchmarks show that SOAR improves performance, yielding gains of up to 7.0% on general reasoning tasks and 16.9% on deep research tasks, while reducing erroneous and inefficient tool usage.

原文摘要

This paper presents AutoRAN, the first framework to automate the hijacking of internal safety reasoning in large reasoning models (LRMs). At its core, AutoRAN pioneers an execution simulation paradigm that leverages a weaker but less-aligned model to simulate execution reasoning for initial hijacking attempts and iteratively refine attacks by exploiting reasoning patterns leaked through the target LRM’s refusals. This approach steers the target model to bypass its own safety guardrails and elaborate on harmful instructions. We evaluate AutoRAN against state-of-the-art LRMs, including GPT-o3/o4-mini and Gemini-2.5-Flash, across multiple benchmarks (AdvBench, HarmBench, and StrongReject). Results show that AutoRAN achieves approaching 100% success rate within one or few turns, effectively neutralizing reasoning-based defenses even when evaluated by robustly aligned external models. This work reveals that the transparency of the reasoning process itself creates a critical and exploitable attack surface, highlighting the urgent need for new defenses that protect models’ reasoning traces rather than merely their final outputs.

Influence-based Online Experience Selection for Effective RLHF

Yifan Gong, Jing Yao Microsoft, Xiting Wang Renmin University of China, Xunlong Wang Renmin University of China, Xiaoyuan Yi Microsoft Research, Xing Xie Microsoft Research Asia

原文摘要

Reinforcement Learning from Human Feedback (RLHF) has emerged as a crucial technique for aligning large language models (LLMs) with human preferences. However, existing RLHF methods face key challenges, including poor sample efficiency, high computational overhead, and slow convergence. Recent studies highlight the importance of data selection in RL, but how to effectively select the most beneficial experiences for RL training remains an open problem. Existing data selection methods for RL rely on heuristic metrics, failing to establish an interpretable connection between data and optimization objectives. To address this problem, we propose InfOES (Influence-based Online Experience Selection), a novel data selection method for RLHF that dynamically estimates the influence of individual training samples on policy optimization. By incorporating data attribution into the policy gradient, InfOES can identify and filter out detrimental samples on the fly, ensuring effective convergence toward alignment objectives. Our approach is compatible with various RL algorithms (e.g., PPO, GRPO, REINFORCE++). Extensive experiments demonstrate that InfOES significantly enhances training effectiveness, achieving superior alignment performance with fewer optimization steps.

原文摘要

Ensuring that Large Language Models (LLMs) adhere to safety principles without refusing benign requests remains a significant challenge. While OpenAI introduces deliberative alignment (DA) to enhance the safety of its o-series models through reasoning over detailed “code-like” safety rules, the effectiveness of this approach in open-source LLMs, which typically lack advanced reasoning capabilities, is understudied. In this work, we systematically evaluate the impact of explicitly specifying extensive safety codes versus demonstrating them through illustrative cases. We find that referencing explicit codes inconsistently improves harmlessness and systematically degrades helpfulness, whereas training on case-augmented simple codes yields more robust and generalized safety behaviors. By guiding LLMs with case-augmented reasoning instead of extensive code-like safety rules, we avoid rigid adherence to narrowly enumerated rules and enable broader adaptability. Building on these insights, we propose CADA, a case-augmented deliberative alignment method for LLMs utilizing reinforcement learning on self-generated safety reasoning chains. CADA effectively enhances harmlessness, improves robustness against attacks, and reduces over-refusal while preserving utility across diverse benchmarks, offering a practical alternative to rule-only DA for improving safety while maintaining helpfulness.

PAM: Enhancing General Alignment of Large Reasoning Models through Priority-Aware Metacognition

Zhihao Xu, Fuzhen Yang, Liang Lin, Xiting Wang Renmin University of China

原文摘要

Recent advancements in Large Reasoning Models (LRMs) have showcased strong performance across various reasoning tasks by leveraging System-2 thinking capabilities. However, existing studies indicate that this reasoning ability alone does not reliably transfer to the general alignment domain. Inspired by cognitive science and how humans solve tasks, we argue that LRMs must be equipped with metacognitive knowledge to fully utilize their System-2 capabilities. In this paper, we propose Priority-Aware Metacognition (PAM), which guides the model to first identify the top-level human preference (e.g., harmlessness) as a means of understanding the alignment task’s nature, and then apply other kinds of metacognitive knowledge to better monitor and regulate the model’s thinking process. We implement PAM via a two-stage pipeline: a cold-start phase that collects structured metacognitive knowledge based on Flavell’s theoretical framework, and a preference-optimization phase that further reinforces such metacognition. Extensive experiments validate the effectiveness of PAM. Under the same training pipelines, PAM consistently yields higher performance, improving general domain alignment performance by ~10 points on the helpfulness and harmless benchmarks. Code is available at https://anonymous.4open.science/r/PAM-RM-02DF.

Unlocking Implicit Experience: Synthesizing Tool-Use Trajectories from Text

原文摘要

Enabling Large Language Models (LLMs) to effectively utilize tools in multi-turn interactions is essential for building capable autonomous agents. However, acquiring diverse and realistic multi-turn tool-use data remains a significant challenge. In this work, we propose a novel text-based paradigm. We observe that textual corpora naturally contain rich, multi-step problem-solving experiences, which can serve as an untapped, scalable, and authentic data source for multi-turn tool-use tasks. Based on this insight, we introduce GEM, a data synthesis pipeline that enables the generation and extraction of multi-turn tool-use trajectories from text corpora through a four-stage process: relevance filtering, workflow tool extraction, trajectory grounding, and complexity refinement. To reduce the computational cost, we further train a specialized Trajectory Synthesizer via supervised fine-tuning. This model distills the complex generation pipeline into an efficient, end-to-end trajectory generator. Experiments demonstrate that our GEM-32B achieve a 14.9% improvement on the BFCL V3 Multi-turn benchmark. Our models partially surpass the performance of models trained on -bench (Airline and Retail) in-domain data, highlighting the superior generalization capability derived from our text-based synthesis paradigm. Notably, our Trajectory Synthesizer matches the quality of the full pipeline while significantly reducing inference latency and costs.

From Verbatim to Gist: Distilling Pyramidal Multimodal Memory via Semantic Information Bottleneck for Long-Horizon Video Agents

原文摘要

While multimodal large language models have demonstrated impressive short-term reasoning, they struggle with long-horizon video understanding due to limited context windows and static memory mechanisms that fail to mirror human cognitive efficiency. Existing paradigms typically fall into two extremes: vision-centric methods that incur high latency and redundancy through dense visual accumulation, or text-centric approaches that suffer from detail loss and hallucination via aggressive captioning. To bridge this gap, we propose MM-Mem, a pyramidal multimodal memory architecture grounded in Fuzzy-Trace Theory. MM-Mem structures memory hierarchically into a Sensory Buffer, Episodic Stream, and Symbolic Schema, enabling the progressive distillation of fine-grained perceptual traces (verbatim) into high-level semantic schemas (gist).Furthermore, to govern the dynamic construction of memory, we derive a Semantic Information Bottleneck objective and introduce SIB-GRPO to optimize the trade-off between memory compression and task-relevant information retention. In inference, we design an entropy-driven top-down memory retrieval strategy.Extensive experiments across 4 benchmarks confirm that MM-Mem achieves state-of-the-art performance on both offline and streaming tasks, demonstrating robust generalization and validating the effectiveness of cognition-inspired memory organization.Code and associated configurations are publicly available at ‘https://github.com/EliSpectre/MM-Mem‘.

ICDAGENT: Empowering Agentic Large Language Models for Explainable Medical Coding

Ziyi Yin, Yuanpu Cao Pennsylvania State University, Ting Wang State University of New York at Stony Brook, Jinghui Chen Pennsylvania State University, Fenglong Ma Pennsylvania State University

原文摘要

The explainable medical coding task aims to automatically assign International Classification of Diseases (ICD) codes to clinical notes while providing explicit justifications for each assignment. Recent approaches employ large language models (LLMs) to generate such explanations. However, their performance remains limited due to a lack of understanding of the clinical meanings of ICD codes. Additionally, the vast ICD code space further complicates the task of accurate prediction. To address these challenges, we propose the ICDAGENT framework, which consists of two collaborative LLM agents: a coding agent and a critical agent. The coding agent extracts ICD codes and generates preliminary rationales, while the critical agent performs fine-grained chain-of-thought reasoning to verify and refine them. Furthermore, the critical agent is trained with a rationale-aware reward, combined with reinforcement learning, enabling it to distinguish between correct and incorrect reasoning and ensure explanation accuracy. Experiments across multiple ICD coding standards and datasets demonstrate that ICDAGENT achieves effective ICD coding with accurate and trustworthy explanations.

Reinforcement learning (RL)-based enhancement of large language models (LLMs) often leads to reduced output diversity, undermining their utility in open-ended tasks like creative writing. Current methods lack explicit mechanisms for guiding diverse exploration and instead prioritize optimization efficiency and performance over diversity. This paper proposes an RL framework structured around a semi-structured long Chain-of-Thought (CoT), in which the generation process is decomposed into explicitly planned intermediate steps. We introduce a Diverse Planning Branching method that strategically introduces divergence at the planning phase based on diversity variation, alongside a group-aware diversity reward to encourage distinct trajectories. Experimental results on creative writing benchmarks demonstrate that our approach significantly improves output diversity without compromising generation quality, consistently outperforming existing baselines.

原文摘要

Retrieval-augmented generation (RAG) is a widely adopted paradigm for enhancing LLMs in medical applications by incorporating expert multi-modal knowledge during generation. However, the underlying retrieval databases may naturally contain, or be intentionally injected with, adversarial knowledge, which can perturb model outputs and undermine system reliability. To investigate this risk, prior studies have explored knowledge poisoning attacks in medical RAG systems. Nevertheless, most of them rely on the strong assumption that adversaries possess prior knowledge of user queries, which is unrealistic in deployments and substantially limits their practical applicability. In this paper, we propose M^3Att, a knowledge-poisoning framework designed for medical multimodal RAG systems, assuming only limited distribution knowledge of the underlying database. Our core idea is to inject covert misinformation into textual data while using paired visual data as a query-agnostic trigger to promote retrieval. We first propose a unified framework that introduces imperceptible perturbations to visual inputs to manipulate retrieval probabilities. Besides, due to the prior medical knowledge in LLMs, naively poisoned medical content with explicit factual errors can be corrected during generation. Thus, we leverage the inherent ambiguity of medical diagnosis and design a covert misinformation injection strategy that degrades diagnostic accuracy while evading model self-correction. Experiments on five LLMs and datasets demonstrate that M^3Att consistently produces clinically plausible yet incorrect generations. Codes: https://anonymous.4open.science/r/M3Att.
